The Marshfield Medical Center-Dickinson (DCHS) seeks a full-time RN Coordinator for Outpatient Quality & Chronic Care to join their team in Iron Mountain, Michigan.
This position manages team-based care to individuals through partnerships with patients and their caregivers/families, healthcare providers, and community resources. The ideal candidate is able to identify, track, and manage the chronically ill patient population especially in the clinic setting.
RESPONSIBILITIES.
- Manage patient access to appropriate medical/specialty providers and community resources by assessing patient/family’s unmet health and social needs, responding to patients with empathy and concern, providing effective communications to improve health literacy, and acting as a patient advocate.
- Promote healthy behaviors in all populations.
- Manage/attend meetings between patients, families, care teams, payors and community resources utilizing tools which support a guided care process. Create ongoing processes for patients and families to determine and request the level of care coordination support they desire over time.
- Participate in committees related to quality care (e.g., ACO/CIN, Quality Improvement Committee, Pediatrics Focus, Post-Acute Care, etc.) and report care coordination activities.
- Utilize IT resources and patient databases to collect data on areas of high risk, high volume, and problem-prone areas of care coordination in order to improve quality of care. Assist with chart abstraction for ACO programs as needed. Track and tend data; make recommendations.
- Focus on care coordination, financial management, and resource utilization to include utilization review, clinical documentation improvement, and discharge planning.
WAGE. Starting pay of $30.40 – $40.21 per hour based on experience plus a $2,500 sign-on bonus.

SCHEDULE. This is a full-time Monday-Friday day-shift position with start times between 7:00am-8:00am. No weekends.
MINIMUM AND PREFERRED QUALIFICATIONS.
- Licensed Registered Nurse in the State of Michigan required.
- Minimum of 1 year clinical or hospital experience required.
- Basic Cardiac Life Support (BCLS) certification, or ability to obtain within 30 days if hired for this position.
- Experience caring for those with chronic diseases, understanding of the chronic care model and evidence-based care, discharge planning, and care coordination highly preferred.
- Ability to perform comprehensive patient assessments to identify case management needs.
- Ability to manage a diverse workload and prioritize tasks.
- Exceptional communication skills with a professional demeanor that allows you to advocate for and ensure trust, competence and compassion amongst patients, families, providers and team members.
- Ability to successfully pass a pre-hire drug screening and background check.
